Fundamental Surgery is driving change in the world of medical training, with virtual reality surgical simulations - featuring haptic touch, plus real-time assessment, on compact, low-cost hardware.

We believe that every surgeon should have the opportunity to rehearse, practise and test themself within a safe, controllable space that is as close to real life as possible and is within arm’s reach of their work place.

The benefits of surgical simulation are well documented; more hands-on experience improves technical practice; and technical practice improves patient outcomes.

The problem is cost.  Cadaveric training is expensive and not always practical.  And with traditional simulations, the capital investment is often heavy.  As a result, access to surgical simulation for most surgeons is rare.

Fundamental Surgery is a software platform that takes advantage of readily available VR hardware to create a simulation environment that can be used on any modern PC.

  • Total Knee Arthroplasty
    Perform a virtual total knee arthroplasty. Fundamental Surgery uses the latest high definition graphics and virtual reality techniques combined with the sense of touch to deliver a ‘feel real’ experience....

  • You will prepare the knee to receive the femoral, tibial and patellar implants. Using a surgical drill and sagittal saw you will know what it feels like to make all the key resections of the femur, tibia and patella. By applying and setting cutting guides for each resection, you will pay keen attention to the cutting angles required to maintain the correct anatomical and mechanical axes of the leg. You will learn the key depths and sizes of cuts that are required and how these are matched to the implants for your patients’ anatomy, ultimately resulting in better patients’ outcomes. You will learn how to prepare the tibia and patella to receive the implants using the tibial stem punch and patellar drill guide. You will be assessed on the key cuts with respect to the angles, depths and protecting the key lateral, medial and posterior structures of the knee from damage. Finally you will see how the implants are applied to complete the arthroplasty.

  • Spinal Pedicle Screw
    Perform a virtual Spinal Pedicle Screw. Fundamental Surgery uses the latest high definition graphics and virtual reality techniques combined with the sense of touch to deliver a ‘feel real’ experience....

  • You will prepare the thoracic and lumbar vertebrae for pedicle screw insertion using a variety of tools such as the rongeur, pedicle awl and pedicle probe. The prepared pedicle channel’s integrity will then be checked with a pedicle feeler, before finally inserting the pedicle screws for each vertebrae. Throughout the procedure you can use the image intensifier to check the progress of the channel and be sure you are not breaching the pedicle walls. The key learning is focused on the specific sagittal and transverse angles and screw depths required for each pedicle channel along the spine.

  • Total Hip Arthroplasty
    Perform a virtual total hip arthroplasty. Fundamental Surgery uses the latest high definition graphics and virtual reality techniques combined with the sense of touch to deliver a ‘feel real’ experience....

  • You will re-sect the left femoral head with a sagittal saw, prepare the acetabulum with a reamer and fit the acetabular cup and liner. Then you will focus on preparing the femur to receive the stem implant with a series of tools including a box osteotome, femoral reamer and femoral broaches. The stem implant and prosthetic head will then be applied, the hip reduced into the acetabular cup and the patient’s leg length checked. The key learning is focussed on ensuring an appropriately angled resection of the femoral head, an appropriate placement for the acetabular cup and liner, and appropriate location of the stem implant within the femur.
